Older adults are not one risk group. A healthy 66-year-old using one medicine and an 86-year-old with balance problems, kidney disease and eight prescriptions need different plans. Product-specific dosing and follow-up are more useful than a general promise that cannabis is gentle.

What the adverse-event evidence shows
A 2024 systematic review and meta-analysis examined randomized trials of cannabinoid-based medicines in adults whose mean age was at least 50. THC-containing treatments were associated with dose-related increases in several adverse events, including dizziness, lightheadedness and balance or coordination problems. The studies involved different products and conditions, so the results do not supply one universal safe dose.
That pattern is clinically important because a brief episode of dizziness can become a fall, fracture or loss of independence. Risk may rise with antihypertensives, sedatives, alcohol, poor vision or an unfamiliar nighttime route through the home.
Choose a measurable goal
“Feel better” is hard to evaluate. A useful plan might target two fewer nighttime awakenings, easier dressing despite spasticity or a one-point reduction in pain while preserving morning alertness. Track the goal alongside adverse effects for at least several days before changing the dose, unless a clinician advises otherwise.
Route changes timing
Inhaled THC acts quickly, which makes onset easier to recognize but introduces pulmonary exposure. Oral products can take much longer and last longer, making early redosing a common mistake. A caregiver should know the product, dose and expected window, especially when memory problems are present. Store products away from children and from ordinary candy or medicine organizers.
A practical first-dose plan
- Review every prescription, over-the-counter drug and supplement with a pharmacist or clinician.
- Start when driving, stairs and caregiving duties can be avoided.
- Use one clearly labeled product and record the THC and CBD milligrams.
- Do not redose an edible because nothing happened in the first hour.
- Check standing dizziness, confusion, gait and next-morning function.
